How it works

Your care, step by step.

From your first questions to medication at your door and ongoing follow-up — here's exactly what to expect with Forma.

  1. Complete your intake

    Today · ~5 min

    Answer a few health questions, share your goals, and sign your telehealth consent. Takes about 5 minutes.

  2. Pay the consultation fee

    $120 · one-time

    A one-time $120 fee reserves your video visit with a licensed Forma clinician. This covers the consultation only — not medication.

  3. Meet your clinician by video

    TODO: typical wait to first visit (e.g. within 2–3 days)

    Choose a time that works for you and meet one-on-one over a secure video visit. Your clinician reviews your intake, BMI, and history to decide what care is appropriate for you.

  4. Your prescription goes to the pharmacy

    If your clinician determines treatment is appropriate, they send your prescription to a licensed pharmacy. Booking a visit does not guarantee a prescription.

  5. Pay for your medication

    Medication is billed separately from the consultation fee. TODO: explain how patients pay for medication (pharmacy, insurance, or cash price) and typical cost ranges.

  6. Your medication ships to your door

    The pharmacy ships your medication discreetly to the address on your file. TODO: confirm shipping timeline and carrier.

  7. Ongoing follow-up & support

    TODO: follow-up email cadence (e.g. check-in at week 2, then monthly)

    We check in on your progress and your clinician adjusts your plan as needed. You can reach the care team any time by phone or text. TODO: confirm the follow-up schedule.
